CALEDON FLAG FOOTBALL LEAGUE

• I, the undersigned, acknowledge that I am the parent or legal guardian of the child named above, who will be participating in flag football activities, programs, games, practices, events, and related activities (collectively, the "Activities") operated by Caledon Flag Football League Inc. ("CFFL").

• I acknowledge and accept that participation in the Activities involves certain risks, both known and unknown, including but not limited to illness, injury, accident, contact with other participants, weather-related conditions, and other risks associated with athletic activities.

• I understand that CFFL, including its associated organizations, staff members, employees, volunteers, officers, directors, agents, representatives, and any owners or occupiers of premises used in connection with the Activities, are not responsible for any illness, injury, loss, damage, or expense that may occur as a result of my child's participation in the Activities.

• I hereby release and forever discharge CFFL, its affiliated entities, officers, directors, employees, agents, volunteers, representatives, sponsors, facility owners or occupiers, and any other persons or organizations acting on behalf of or in connection with CFFL (collectively, the "Releasees") from any and all claims, actions, causes of action, losses, damages, expenses, illness, injury, or death arising from or related to my child's participation in the Activities, whether caused by negligence, accident, or otherwise.

• I confirm that, to the best of my knowledge, my child is physically, mentally, and emotionally able to participate in the Activities. I agree to disclose any medical conditions, allergies, injuries, or other relevant information that may affect my child's participation.

• I confirm that, to the best of my knowledge, my child is free from any contagious or communicable illness. If my child becomes ill or there is a risk of illness, I will notify CFFL as soon as possible.

• In the event my child becomes ill, injured, or requires medical attention during the Activities, I authorize CFFL staff or volunteers to take reasonable action they deem necessary for my child's well-being, including obtaining medical treatment if required. I understand that any related costs will be my responsibility.

MEDIA CONSENT

• I grant permission to CFFL to use, reproduce, and/or distribute photographs, videos, audio recordings, or other media of my child for promotional, marketing, social media, website, or other lawful purposes related to CFFL. I waive any right to inspect or approve the final materials and understand that no compensation will be provided for their use.

CODE OF CONDUCT

• I agree that my child and I will comply with all CFFL rules, policies, and conditions as communicated or posted. I understand that participation in the Activities is subject to compliance with these rules.

• I understand that CFFL has a zero-tolerance policy for violence, harassment, bullying, unsafe conduct, or behaviour that places any participant, volunteer, staff member, or spectator at risk. Any such behaviour may result in removal from the Activities without refund, at CFFL's discretion.

CHANGES TO ACTIVITIES

• I understand that CFFL makes every effort to offer each division and schedule as listed. However, divisions, schedules, locations, and Activities may be amended, rescheduled, combined, relocated, or cancelled due to enrolment, weather, field availability, safety concerns, or circumstances beyond CFFL’s control. Based on registration numbers, divisions may also be combined by gender, age group, or co-ed format where necessary.

REFUNDS

• I acknowledge and accept that CFFL may not provide refunds or credits for missed Activities, except where determined by CFFL in its sole discretion.

PRIVACY

• I understand that CFFL may collect, use, and store personal information for the purposes of registration, communication, scheduling, payment processing, safety, and administration of the Activities. CFFL is committed to compliance with applicable privacy laws.
